Scarf & Akin Osteotomy
For Hallux Valgus
What It Involves
A Scarf osteotomy involves making a Z-shaped cut through the first metatarsal bone. The bone is shifted to correct the angulation and fixed with small titanium screws. An Akin osteotomy is a small wedge cut in the base of the big toe bone to fine-tune alignment. Together these achieve a balanced, durable correction of the bunion deformity.
The operation is performed under general or regional anaesthesia with a tourniquet and typically takes 60–90 minutes.
Recovery Timeline
Everyone heals at a different rate — this timeline is a guide.
Weeks 1–2
A surgical sandal is worn, with full weight-bearing through the foot as comfortable. The wound is checked and sutures removed at 2 weeks.
Weeks 2–4
Continuing in the surgical sandal as swelling gradually settles. Sedentary or desk-based work is often possible from this point.
Weeks 4–6
Transitioning out of the surgical sandal into wide, comfortable shoes. An X-ray is taken at 6 weeks to confirm healing.
Weeks 6–12
Continuing transition into normal footwear, with physiotherapy for range of motion and strengthening. Jobs requiring prolonged standing or walking typically need 6–12 weeks off.
3–6 months
Return to sport from around 3–6 months, with high-impact activity from around 4–6 months. A functional review and discharge typically takes place at the 3-month mark.
